The title serves double meaning: apparent and underlying. The apparent meaning is of course about the physical appearance of Lindsay which does not match the Upper East Side ladies who are thin, one-dimensional and blonde. But the underlying meaning would be that she is an outsider(by her own choice?); a misfit in the shallow world of trophy wives.
